I will check with my insurance provider to determine if additional insurance coverage is needed to cover me and other participants in my family group while attending GENEVA or participating in activities at GENEVA.
Unless otherwise disclosed to GENEVA, I give permission for my child or other minor over whom I have responsibility to engage in all prescribed camp activities and to be photographed and videoed by GENEVA, which photographs and videos may be used at GENEVA’S discretion in camp publicity.
I am responsible for the supervision and care of my child(ren) and other minors in my family group while at Family Camp.
